REQUIRED QUALIFICATIONS:  Registered nurses will be graduates of an accredited school of nursing and hold a current valid RN license to practice in the State of New York. 

MAJOR FUNCTIONS: The Registered Nurse functions as the patient care coordinator by developing adequate means and methods to facilitate implementation of patient care policies and procedures.

1.     Initiates nursing process utilizing the nursing assessment tool on an ongoing basis, by planning coordinating, implementing and evaluating nursing care based on clinical knowledge, patient's needs and a collection of data
2.     Develops and updates treatment plans to address nursing diagnosis and reviews with the multidisciplinary team to provide input for diagnosis, length of stay and readiness for the next level of care.
3.     Implements detoxification procedure and delivers appropriate patient care according to withdrawal scale and physicians orders.
4.     Retrieves orders and administers correct medications per physician’s orders and facilitates procedure and policy.  Maintains update knowledge of withdrawal medications and modalities used in addiction treatments.
5.     Records and reports to appropriate care providers any information that may impact the patient's treatment and/or continuing care and completes medical discharge form.
6.     Initiates consult(s) (psych, nutritional) when appropriate.
7.     Assesses patient for readiness for transfer to next level of care. 
8.     Triage to minor injuries and illnesses of patients in coordination with the medical staff and RN Charge Nurse. 
9.     Maintains confidentiality at all times.
10. Assumes responsibility for growth and accountability through in-service programs and staff meetings.
11. Assists in orientation for new employees and/or serves as preceptor.
12. Reports for duty as scheduled and is capable of taking charge after completing charge orientation. 
13. All information maintained accurately and in compliance in electronic chart.

REQUIRED QUALIFICATIONS: HS diploma or GED.  Must be able to lift 50 lbs occasionally.

MAJOR FUNCTIONS: To assist in the preparation and all tasks of cafeteria style meal service for 155 patients plus staff 

1.     Follows all mandates and procedures as outlined in the operations manual concerning food preparation, services storage and sanitation as set forth by the New York State Department of Health and Arms Acres.
2.     Sets up sanitation and food service stations in both kitchen and dining room areas.
3.     Cleans, rotates stock and organizes service lines, coolers and storage areas inside and outside of the kitchen, frequently lifting and/or moving up to 25 pounds and occasionally lifting and/or moving up to 75 pounds with assistance.
4.     Takes inventory, reports shortages or missing items in the kitchen; assembles and delivers supplies for all wing kitchens.
5.     Stocks and prepares all items necessary for salad bar set up including drainage and sanitation of the salad bar unit.
6.     Prepares, assembles, delivers, and retrieves meal trays and/or supplies for all unit patients as called for.  Special attention is given to keeping cold foods cold and hot foods hot.
7.     Satisfactorily completes all designated assignments set forth in the daily production sheet package.
8.     Sets up, breaks down and cleans for special functions as needed.
9.     Assists other Dietary Staff as needed or directed. Employees are expected to use teamwork to accomplish tasks that require extra exertion above the ability of one individual.

JOB: HIM TECHNICIAN             
DEPT: HEALTH INFORMATION MAINTENANCE  
SUPERVISOR: DOLORES WATSON                                        
STATUS: TEMPORARY
                                             
HOURS: 40
REQUIRED QUALIFICATIONS:  High School diploma or GED plus six months to one year related experience and/or training. Typing skills @ 40 WPM or more with excellent accuracy. Excellent listening, spelling & grammar skills. MS Access, Excel & Meditech a plus.
MAJOR FUNCTIONS: Medical records management; including correspondence, mandated reporting and other HIM office work.

 1. Maintains confidentiality of patients in accordance with federal rules found at 42 CFR Part 2, 45 CFR and NYS Public Health Law.
2. Handles written correspondence requests according to procedure. Processes phone calls, both internal and external, from personnel in regards to record requests. Enters routine correspondence requests for discharge summaries to be sent to referent, aftercare provider, and probation/ parole immediately following discharge.
3. Generates letters according to procedure.
4. Processes disability paperwork according to procedure.
5. Follows up with Utilization Review and Business Office regarding records for insurance appeals.
6. Provides clerical support to case managers, utilization review, business office, regional services, doctors, and administrative personnel.
7. Pulls and files medical records according to procedure.
8. Files loose papers in appropriate patient files. Maintains files in numerical order. Removes from and adds flags to folders appropriately.
9. Boxes closed charts for transfer to Iron Mountain. Completes transmittal sheets for Iron Mountain archiving that are accurate and timely.
10. Audits closed charts according to procedure. Logs deficiencies and corrections in an accurate and timely manner. Generates accurate lists of chart deficiencies from the database on an as needed basis and distributes weekly.
11. Signs paper charts in and out of the office in Meditech.
12. Proficient in all aspects of Meditech, 10e11, MS Access.
13. Works with office coordinators at all sites and Transportation to collect charts and route back and forth as needed according to procedure. Follows chain of custody procedure when receiving and transporting charts.
